Dear family member who is also a doctor--
When I mention that my neck hurts, maybe you could look at it? Instead of saying "heavy people have all sorts of issues". Because I don't think that my head is heavy. But of course I'm not a doctor.

THE HUMAN HEAD WEIGHS EIGHT POWNDS
You should go to medical school and become a doctor who specializes in neck problems. And then when he says "heavy people have neck problems" you can say "the actual medical term is 'Clinically obese people have cervical disc issues. I can't believe you didn't learn that in medical school"
Then he'll be so embarrassed that he'll diagnose your neck problem for free.
You must be referring to my brother the pompous ass doctor. See you at the next family reunion. We'll meet at the "heavy people with issues" table right next to the dessert.
Wow. What can I say but I'm sorry you have an idiot for a family member. And he's a doctor! His compassion level is just plain scary.
What an ass.
